If you’re taking tadalafil as needed for erectile dysfunction, talk with your doctor about the timing of your dose. You may be able to have sexual activity after 30 minutes and up to 36 hours after taking your tadalafil dose. In this case, the recommended starting dose of tadalafil is 40 mg. You’ll likely take two 20-mg tablets at once for your daily dose of 40 mg.
By inhibiting PDE5, tadalafil leads to an increase in the levels of cyclic guanosine monophosphate (cGMP), which causes relaxation of smooth muscle and increased blood flow to specific areas of the body. In the case of ED, this results in improved erectile function by enhancing the blood flow to the penis upon sexual stimulation. For PAH, tadalafil helps to relax and widen the blood vessels in the lungs, which lowers the pulmonary blood pressure and improves symptoms. Alcohol and PDE5 inhibitors, including tadalafil, are mild systemic vasodilators.

In patients with mild or moderate hepatic impairment, the dose of CIALIS should not exceed 10 mg. Because of insufficient information in patients with severe hepatic impairment, use of CIALIS in this group is not recommended (see Pharmacokinetics in Special Populations under CLINICAL PHARMACOLOGY).
